Assessment Overview
Your initial GAP visit will include:
- A full physical examination
- Cognitive and other testing
- Review of patient’s preferences, values, and goals
- Review of patient’s medical history and medications
We strongly suggested that the patient bring along a trusted family member or caregiver to the appointment to provide additional information and to help formulate the plan of care.
